File within one year
You must file your complaint within one year of the date the alleged misconduct happened.
Gather the information you need
Before you begin, have the following information ready, if available:
- Full name of the judge, family support magistrate, or administrative law judges
- Court location
- Docket number
- Date the alleged misconduct happened
- Description of what happened
- Copies of supporting documents
Complete the complaint form
Complete the complaint form before you sign it.
Type your responses if possible. If you complete the form by hand, please make sure your handwriting and signature are easy to read. If they are not, your complaint will be denied.
Sign before a notary
Do not sign the complaint form until you are with a notary public or commissioner of the Superior Court.
You must sign your complaint under oath before a notary public or commissioner of the Superior Court before you submit it.
Submit your complaint by mail
Submit your completed and notarized complaint by mail only.
