Use this guide to prepare and send your appeal forms to the Board of Firearms Permit Examiners (BFPE). We explain how to save your forms, how to send them by email or mail, and what to do if something goes wrong.
Download forms before you complete them
Some PDF forms may not save your answers if you complete them in a web browser. Save the file first, then fill it out.
- Download the form to your computer or device: Appeal letter and questionnaire (PDF)
- Complete all required sections
- Save the file before you close it
- Review your saved file to confirm your answers stayed in place
Submit your forms by email
Email is the fastest way to send your appeal forms.
- Attach your completed forms as PDF files
- Include your name and contact information in your message
- Send your forms to: bfpe@ct.gov
- Keep a copy of the email for your records

You can also send your forms by mail.
If you cannot complete the form on your device, you may print it and complete it by hand.

What to do if you have trouble sending your forms
If you have trouble sending your forms, try these steps:
- Check that your file is saved and attached
- Make sure your file opens correctly before sending
- Try sending the email again
- Send your forms by mail if email does not work
