How to use the Form to Appeal Due to the Delay

You can use the Form to Appeal Due to the Delay when 8 weeks have passed and the issuing authority still has not approved or denied your temporary pistol permit application. How to copy the form into an editable document

The form on the website cannot be edited. Please copy the same sections into an editable document and type your answers there. Use the BFPE form and questionnaire as your guide. Include all required information. Copy the form exactly as it appears. Do not change the wording or order of the sections. This helps speed up the review process.

The form requests the following information

  • Your name, address, date of birth, email, and phone number
  • The name of the issuing authority and the date you applied
  • A statement that 8 weeks have passed with no approval or denial
  • A statement that you made a good-faith effort to contact the issuing authority

Submit the completed form through the website. If the online submission does not work, you can send the form by email or mail. The BFPE contact page includes the Board’s email address and mailing address.
