• Contracts - (CHC) Connecticut Home Care

  A listing of contracts associated with Connecticut Home Care.

• Connecticut Home Care Program for Elders (CHCPE)

  This program provides services to help eligible residents live in the community. Eligible applicants must be 65 years of age or older, be at risk of nursing home placement and meet the program’s financial eligibility criteria. To be at risk of nursing home placement means that the applicant needs assistance with critical needs such as bathing, dressing, eating, taking medications and toileting. CHCPE helps clients continue living at home instead of going to a nursing home. Each applicant’s needs are reviewed to determine if he/she may remain at home with the help of home care services.

• Social Work Services

  Social Work Services develops services and methods of service delivery designed to respect the client’s right to self-determination and empower and protect individuals, families and those who are economically disadvantaged or otherwise vulnerable.

• Money Follows the Person Program

  Money Follows the Person is a Federal demonstration project dedicated to assuring Connecticut residents access to a full range of high quality, long-term care options that maximize autonomy, choice and dignity.

• Community First Choice

  Are you or a loved one living in the community and need assistance to remain there? The Affordable Care Act created an optional State Plan service which will allow eligible individuals to access Personal Attendant Care (PCA) and other services and supports through self-direction.

• Connecticut Housing Engagement and Support Services (CHESS) Initiative

  A listing of contracts associated with the Behavioral Health Program.

• Med-Connect (Medicaid for Employees with Disabilities)

  Medicaid for Employees with Disabilities, also known as MED-Connect, provides medical assistance to employed individuals with disabilities. Enrollees can earn up to $75,000 per year and qualify for full Medicaid/HUSKY Health coverage.

• Certified Community Behavioral Health Clinics

  In August 2015 the State of Connecticut, specifically the Departments of Social Services, Mental Health and Addiction Services, and Children and Families, submitted an application for the Substance Abuse and Mental Health Services Administration (SAMHSA) funding opportunity 16-001, Certified Community Behavioral Health Clinics Planning Grant.

• Food Assistance - Farmers' Markets, Farms and Farm Stands

  Many Farmers' Markets and Farm Stands throughout Connecticut now can accept EBT/SNAP (Formerly known as Food Stamps). This means you will once again be able to buy locally grown fruits and vegetables that arrive more quickly and fresher than those transported long distances from other states and countries.

• Non-Emergency Medical Transportation (NEMT)

  Medicaid Non-Emergency Medical Transportation (NEMT) is an important benefit for Medicaid members who need to get to and from Medicaid-covered medical services but have no means of transportation.
